This is the correct place to be if you’ve had your pre-procedure consultation with Dr. Tonozzi and it’s time to confirm the appointment for your no-needle, no-scalpel vasectomy.  Below, you’ll be reviewing the consent form for the procedure.  Please let us know if you have questions prior to signing it.  You’ll also be submitting your payment information.  Your card will not be charged until the day of the procedure.  Our cancellation policy: if you cancel at least 2 days in advance, you will get a full refund. If you cancel less than 2 days in advance, we will charge you $100 and refund the balance.

The charge below includes both the procedure and your pre-procedure visit.

A few important reminders:

  • If you’re using pre-procedure medication, pick it up a few days in advance so if there are issues, we can problem solve them. You’ll need somebody to drive you to and from the procedure if you use these medications. 
  • Shave 2-3 days before the procedure.
  • Be sure to get your compression briefs well before the procedure and wear them to the procedure. You may need to order them on the web a week in advance.
  • Shower before the procedure.
  • Have a couple ice packs at home ready to use for the first day after the procedure.

Consent for Procedure. I am aware of the following risks.

I will later decide I want a pregnancy, but I will be unable to reverse the procedure. The reversal surgery is expensive, and it is sometimes not successful. *
That surgery cannot be completed. The shape of the scrotum for some men makes no-scalpel vasectomy difficult. There is a risk that one or both of your tubes cannot be located and divided. The risk of this is higher when you’ve had a pre-procedure consultation by video, since an exam is not done at these visits. About 1 in 200 men will have this problem. *
Pain. It is normal to have pain for the first 2-3 days, but some men have pain that lasts many months. This is referred to as “Chronic Post-vasectomy Pain Syndrome.” About 1 in 250 men will have this. *
Swelling. Some swelling is normal, but some men will have unusual swelling that only subsides with time. It may be painful. About 1 in 50 men will have this problem. *
Allergic reaction to the numbing medication. *
Infection in the tissues of the scrotum *
Failure. The surgery may not succeed in separating both tubes. Also, the tubes may develop a connection later in life. The risk of this is small, about 1 in 1000 men. *
The need to alter the planned procedure. If this happens, I give permission to my doctor to alter the procedure as necessary, or to administer additional anesthetics or other medication if I should need them, for the completion of my procedure. *
